After you book your appointment, you’ll receive an intake form where you can provide your insurance details for a seamless billing process.

Please note: Some insurance providers or plans do not allow me to direct bill. In these cases, I will provide you with a receipt to submit for reimbursement.
How to Use Your Benefits
Once your appointment is booked, I’ll send you an intake form where you can enter your insurance information. A credit card is required to reserve your appointment. After your session, I’ll process the claim directly with your insurance provider. If any balance remains, it will be charged to the card on file. A receipt will be emailed to you for your records.
What Information Do I Need to Provide?
Insurance provider name (e.g. Canada Life)
Policy or plan number
Member ID number
If you're using a spouse or parent’s plan, the plan member’s date of birth
Not Sure if You're Covered?
I recommend contacting your insurance provider in advance to confirm your coverage. Here are a few helpful questions to ask:
Does my plan cover services from a Registered Dietitian and/or Acupuncturist?
How much is covered per year?
Does the plan cover 100% of the session, or is there a per-visit maximum?
When does my coverage year reset?
